Community Health Assessment Kick-off
Please join us at the official kick-off for the 2020-2025 Community Health Improvement Plan!
This session will set the shared vision to guide the Mobilizing for Action through Planning and Partnerships (MAPP) process. Involvement from community businesses, local government, medical providers, funders, organizations, churches and other leaders is crucial to the success of the project and the final plan’s relevance to our community. Every 3-5 years, the community with coordination by the Health Department looks at the current health status of residents. The MAPP committee evaluates that information and its implications. It then identifies priorities and develops a strategic plan to improve those outcomes.
The vision developed at this event will help guide the priorities in the
creation of the new 3-5 year health improvement strategic plan for St. Lucie County to improve the health of those who live, work and play here.
Other sessions will be held over the course of the next 12 months to collect and analyze data, conduct focus groups and gather survey data. We hope you will join us.
