Community Health Improvement Plan

The Healthy St. Lucie Community Health Improvement Plan outlines the shared goals and strategies developed by community partners to improve health outcomes across St. Lucie County.

The plan was created using local health data, community surveys, and collaboration between healthcare providers, nonprofit organizations, educators, government agencies, and residents.
